General comments
Ernstige malaria tropica:
- Asexuele P. falciparum parasietenindex ≥ 5%
- Asexuele P. falciparum parasietenindex < 5% maar met delingsvormen of met één of meer complicaties (overleg dienstdoende internist tropencentrum)
- Ook: bij brakende patiënt!
NB 1 voor artesunaat is artsenverklaring noodzakelijk
NB 2 parasitemie 2-5% is het grijze gebied. Bij een parasitemie 2-5%, starten met iv artesunaat en indien klinische conditie dit toelaat z.s.m. over op orale antimalaria medicatie.
NB 3 tot enkele weken na het toediening van artesunaat voor ernstige malaria is hemolyse beschreven. Daarom wordt geadviseerd wekelijks te controleren op hemolyse tot minimaal 4 weken na het begin van de behandeling.
